RETRAIT DES ASSURANCES – Automne 2011

Bonjour !

Comme vous le savez peut-être, en tant qu’étudiant(e) de l’UQO vous bénéficiez d’une couverture d’assurance collective. Présentement, c’est la période de retrait des assurances et d’inscription familiale pour les nouveaux étudiants inscrits à l’hiver SEULEMENT.

DEMANDE DE RETRAIT DES ASSURANCES

Si vous êtes couvert par une autre assurance santé et dentaire, vous pouvez vous retirer de chacune des parties du régime collectif.  La période de retrait se tiendra du 10 janvier au 4 février 2011.  Seuls les NOUVEAUX étudiants/NOUVELLES étudiantes peuvent soumettre une demande de retrait.

Un remboursement vous sera alors retourné par la poste par l’ASEQ dès réception de votre preuve d’assurances. En consultant le site www.santeetudiante.com, vous trouverez  :

- Inscrire le nom de l’UQO
- Cliquez sur Changement de couverture (COLONNE DROITE)
- Cliquez sur Changer votre couverture
- Cliquez sur Retrait en ligne (COLONNE GAUCHE)

Remplir le formulaire en ligne et surtout ne pas oublier de faire parvenir une copie de votre preuve d’assurance à l’adresse mentionnée sur le formulaire.

Appeler le Régime collectif de l’AGE-UQO au numéro sans frais : 1-877-795-4425.

INSCRIPTION FAMILIALE

Cette couverture vous permet d’ajouter les membres de votre famille (conjoint et / ou enfants).  Les frais pour inscrire un conjoint ou un enfant sont de 173.00$, alors que les frais pour inscrire un conjoint et un nombre illimité d’enfants sont de 346.00$.  Les inscriptions familiale se feront donc a partir du 10 janvier au 4 février 2011. Seuls les NOUVEAUX étudiants/NOUVELLES étudiantes peuvent soumettre une demande d’inscription familiale.

En consultant le site www.santeetudiante.com, vous trouverez  :

- Inscrire le nom de l’UQO
- Cliquez sur Inscrivez votre famille (COLONNE DROITE)
- Cliquez sur Inscription en ligne
- Ouvrez une session et suivez les étapes.

As you may know, students from UQO all benefit from a collective insurance. Up until October 1st, it is the Withdrawal and Couple & Family enrolment period.

Opting out / Withdrawals

If you’re already covered by an equivalent health and/or dental plan, you can opt out and receive a cheque for the amount of the Plan. The Change-of-Coverage Period is from January 10^th to February 4^th, 2011. Only the new students at the winter session can submit a withdrawal request at this time.

A cheque will be then sent to you in the mail by the ASEQ once they have received your proof of insurance. You will find the online process here www.santeetudiante.com,:

- On the first page, write UQO
- Click on English (UP TOP)
- Click on OPT OUTS (RIGHT COLUMN)
- Click on OPT OUT NOW (TOP LEFT)

Login and follow the steps onscreen and do not forget to send the ASEQ a proof of insurance to the address on the form.

COUPLE AND FAMILY ENROLMENT

Your Plan gives you the option to enrol your family (spouse and/or dependants) by completing an enrolment process and by paying an additional fee, over and above your individual fee as a Plan member. The fees to register a member of the family (spouse and/or dependants) are $173.00 and the fee to register an unlimited number of children is $346.00. The couple and family enrolments will begin on January 10^th and will end on February 4^th, 2011. Only the new students at the winter session can submit a withdrawal request at this time.
